Tip: I, the undersigned (for self or as parent/guardian for the minor child above), do acknowledge and understand that I'm assuming the risk of physical injury by participating in Mega Sports Camp at Beach Fellowship. I release Beach Fellowship and its representatives from any claim for injury that I may sustain during the camp. In order that I may receive the proper medical treatment in the event of jury, I hereby authorize the church/camp staff to obtain or provide medical treatment. I hereby hold the Beach Fellowship and the MEGA Sports Camp staff harmless in the exercise of this authority. I further acknowledge and understand that I am responsible for any medical bills that may be incurred on behalf of injuries sustained during camp. I further acknowledge and understand that I am responsible for abiding by the rules and regulations of the camp. PHOTO POLICY: I understand that photos or videos may be used in presentations which are shared with camp families, the church congregation, and possibly online. Finally, I understand that I should bring all reservations/concerns immediately to the director’s attention.
